Influenza is a seasonal condition that causes coughing, sneezing, mild fever and aches in most cases. However, it can sometimes take a serious turn, leading to hospitalization, especially for young children, adults over 65 and pregnant people. A recent study published in JAMA Network Open examined whether the high-dose inactivated influenza vaccine (HD-IIV), which contains four times as much antigen as the standard dose, offers superior protection against hospitalization and death.
Researchers analyzed data from eight large-scale clinical trials involving more than 600,000 participants that compared a high-dose flu shot with the standard flu shot in older adults.

People vaccinated against influenza that season were 40% less likely to test positive for flu than unvaccinated people, according to a study of more than 1.1 million Californians.
Adults aged 65 and older who tested positive for flu were 29% less likely to die of an influenza-related cause if they had received the vaccine, according to the study, published yesterday in JAMA Network Open. The study included flu-related deaths within 30 days of testing.
“These findings are consistent with protection against severe and fatal influenza among people vaccinated against influenza,” the study authors wrote.

High-Dose Flu Shot Beats Standard Dose in Older Adults
Among older adults, high-dose inactivated influenza vaccine (HD-IIV) was associated with lower risks for hospitalizations for influenza, laboratory-confirmed influenza, pneumonia or influenza, cardiorespiratory disease, and all causes than standard-dose IIV (SD-IIV).
“Overall, these findings demonstrate the clinical benefits of HD-IIV in lowering hospitalization risk, underscoring its value in reducing influenza burden among older adults and the corresponding impact across health systems,” the authors wrote.
“These findings may inform decision-makers in developing vaccine recommendations and policies,” they added.
The study was led by Kristoffer Grundtvig Skaarup, MD, Copenhagen University Hospital-Herlev and Gentofte, Copenhagen, Denmark. It was published online on May 26, 2026, in JAMA Network Open.
